Declaration of Conformity (COC) shall include but not limited to the following information:

The name, contact information and authorized signature of the third-party laboratory

The name of the company applying for the declaration of conformity

Identification of work shoes or anti-puncture mats (such as product series, body size, SKU, etc.), size, men’s or women’s shoes

Effective date of the declaration of conformity

The number of the test report on which the declaration of conformity is based

declares that after the product is tested according to the test method corresponding to ASTM F2412, the result meets the test requirements of ASTM F2413-18
